  • When can I apply for a health plan?
    We have open enrollment year round with no restrictions. Call us to help you enroll now at our office line at 915.803.4198 or at 915.474.0897, or email alejandra@fivepointsmecplan.com.
  • How can I request a new membership ID card if I lost/damaged mine?
    To request a new member ID, click here to email our staff to request a new membership ID.
  • What are our operating hours?
    Monday to Friday, 7 AM to 5 PM.
  • How can I check if my doctor is in-network?
    Use Five Points' First Health Provider Portal located on our website here to search our network of partners including a variety of modern and accessible providers who are available for almost everything.
  • Can I upgrade, add dependents, or cancel my plan at any time?
    Absolutely YES. In addition, no additional fees to cancel at any time. You can call us at 915.803.4198 or email us at alejandra@fivepointsmecplan.com, and we will be more than happy to assist you.
  • What if I have questions about my claim?
    Please contact our billing and claims office at 915.803.4198 or email our claim specialists at Iris@fivepointsmecplan.com or Norman@fivepointsmecplan.com.
  • How do I get help from Member Services?
    You have options to get the help you need. For personalized service Call the Member Services number at 915.803.4198 between the hours of 7:00 AM to 5:00 PM Monday through Friday. Our representatives are ready to help you.
  • If I leave my job, where can I find information about continuing my health insurance?
    Please call us directly at 915.803.4198 so we may help you continue coverage under COBRA. Let us know you're interested in buying a COBRA (Consolidated Omnibus Budget Reconciliation Act) policy. By federal law, companies with more than 20 employees need to make you aware of your options for buying this coverage.
  • How do I report a name or address change to my health plan?
    Keeping your name and address up to date with your health plan is important. Here's how to report a change: If you have a Five Points plan through your employer: Let your employer know your name or address has changed. Your employer will send this update to us. If you have a plan that you purchased directly from us (not through an employer): Call us at 915.803.4198 or email Alejandra at Alejandra@fivepointsmecplan.com for immediate service.
  • Where can I get a summary of my benefits (SOB) and summary of benefits and coverage (SBC)?
    A summary of benefits (SOB) and summary of benefits and coverage (SBC) shows what your plan covers and your share of the costs. You may be able to find your SOB and SBC online at our website here.
  • How many retail networks do you offer nationally?
    We offer Optum Rx, a national pharmacy network that provides members with access to over 67,000 pharmacies located in 50 states, the District of Columbia, Puerto Rico and the Virgin Islands. The network includes 100% of the independent and chain pharmacies on our secure member site or by calling Member Services at the toll-free phone number on your Member ID card.
